Abstract

|
본 연구는 장애인에 대한 임상간호사의 옹호간호 영향요인을 파악하고, 간호사의 옹호간호 수행을 돕기 위한 기초자료를 제공하고자 실시되었다. 본 연구를 통해 장애인에 대한 태도 중 낙관주의-인권, 심미적 간호역량 중 환자지향성이 임상간호사의 장애인에 대한 옹호간호의 중요한 영향요인임을 확인하였다. 본 연구의 결과를 토대로 하여 임상간호사를 대상으로 체계적인 장애인 인권교육과 대상자의 요구에 따라 간호를 수행할 수 있는 역량교육을 시행한다면 임상간호사의 옹호간호 수행이 증진될 것이라 기대한다. 본 연구의 결과를 근거로 하여 다음과 같은 제언을 한다. 첫째, 본 연구는 일부지역 상급종합병원 간호사만을 대상으로 하였으므로, 다양한 지역 및 병원을 대상으로 한 반복연구가 필요하다. 둘째, 본 연구에서는 다양한 부서에서 근무하는 일반간호사를 대상으로 하였는데, 향후 연구에서는 재활병동과 같이 장애인을 많이 접할 수 있는 임상간호사를 따로 표집하여 연구해 볼 것을 제언한다. 셋째, 옹호간호 수행에 필요한 간호근무환경을 측정하는 도구를 이용하여 조직적 요인과 옹호간호의 관계에 대한 반복연구가 필요하다. 넷째, 본 연구는 임상간호사만을 대상으로 하였으므로, 간호사 외의 병원구성원을 대상으로 한 후속연구가 필요하다. 다섯째, 임상간호사의 옹호간호를 증진시킬 수 있는 프로그램의 개발 및 적용을 통하여 프로그램의 효과를 검증하는 중재연구가 필요하다.

Purpose: Clinical nurses are the ideal health care providers to advocate for vulnerable and underserved populations such as people with disability. This study aimed to understand factors influencing nursing advocacy for people with disability among clinical nurses.
Methods: The subjects of this study were 186 clinical nurses who were working in three hospitals in B and D cities. Self-report questionnaires were used to collect the data. Data were analyzed using IBM SPSS Statistics 25 with descriptive statistics, t-test, one-way ANOVA, Pearson’s correlation coefficients, and multiple regression analysis.
Results: Clinical experience and esthetical nursing competency have positive correlations with nursing advocacy, and optimism-human rights has a negative correlation with nursing advocacy. Factors influencing nursing advocacy for people with disability were identified as patient directivity (β=0.36, p=.001), optimism-human rights (β=-0.18, p=.008) and clinical experience (≥10) (β=0.14, p=.036). The final model consisting of these factors explained 19% of the variance of nursing advocacy (F=14.99, p=.001).
Conclusion: The findings of this study indicate the importance of developing and implementing nursing interventions that can improve patient directivity and optimism-human rights toward people with disability among clinical nurses. These nursing advocacy interventions can be provided as part of continuing education as well as the nursing curriculum.
